Title: Simple enough, the name of your mission.
Level: It can be Open, meaning for anyone who wants it; Special, meaning it's meant for a mole or a specific person; or High, meaning it's for a high-ranking member of the gang or the leader itself.
Difficulty: The difficulty of the mission. Easy, Medium, or Hard.
Personnel: The number of people allowed. Put N/A if it's unlimited. If you want more than a certain number or less than a certain number, just use < or > accordingly.
Place: Where and when the mission happens, of course.
Goal: One sentence summarizing the objective of the mission.
Description: Here's where you describe the mission in full. What are the players directed to do? What sort of environment are they in? Describe everything important, be detailed, but leave enough room for the players to be creative. Think of it less like a railroad track and more like a car with a nice, wide road to get to where they want to be.
